Prior to admission to Primary Colors EduCare all forms must be filled out and submitted to your child's file.  We will need copies of your child's individual shot records and a note from their physician if they are under care as to their restrictions if any.  Our goal is to not only develop a great relationship with your child, but with you the parent as well.  I will do my part to keep communication lines open and if you are in need of resources or referrals, please do not hesitate to ask, I am here for you also.
